Pernicious Anemia occurs when there is a deficiency of Vitamin B-12.

The patient may have an healthy amount of B-12 in their system, but the parietal cells in their body have been destroyed by an autoimmune disease. Because of that, the body cannot absorb the vitamin, so it never gets to the cells.
